Rubicon is dedicated to guaranteeing that financial constraints will not prevent anyone from accessing medical or dental care.

The Sliding Fee Program is designed to reduce the cost of services for you and your family at Rubicon. We strongly encourage you to apply for this program, even if you currently have insurance coverage.

Reduced Costs for Patient Care

Our Sliding Fee Program offers reduced rates on office visits and services at Rubicon clinics for eligible individuals and families. Once you apply and are approved at a Rubicon location, you will be granted the relevant discount.

This program operates based on household income and has the potential to decrease your medical expenses by 20%, 40%, 60%, 80%, or even more. Eligibility extends to you whether or not you possess insurance—and even if you are enrolled in Medicare or Medicaid.

To enroll in the Sliding Fee Program, kindly furnish one acceptable document verifying income for every individual within your household (including both relatives and non-relatives), as indicated below.

Acceptable Proofs of Income:

  • Two most recent pay stubs

  • Current Tax Return

  • Current W-2s from all employment

  • Bank statements supporting income declaration on sliding fee application

  • Social Security or Unemployment Award Letter

  • Department of Social Services Benefit Statements

  • Letter from a government office explaining the benefits you do or do not qualify for (unemployment office or social security office, etc.)

  • Denial letter for an application of benefits (must be dated within the last 3 months)

Individuals who state they have no income but cannot provide documented proof will need to self-attest, reapply, and submit a complete sliding fee application at every visit.

Sliding Fee Program Eligibility

Eligibility for the program is exclusively decided by your household size and total cumulative income. Should you meet the program’s criteria, the fee for your visit will be lowered in accordance with our established discount schedule.

Sliding Fee Program FAQ

Yes. Should you submit two recent and consecutive pay stubs with your application, the discount will be applied retroactively for the 90 days preceding the application date. If tax statements are utilized, the discount will be effective from the start of the year. This allows you to receive a sliding fee discount for any previous visits within that period.

No. The cost of care remains the responsibility of all patients, including those without insurance.

No, you do not. All are welcome!

If you don’t pay your portion of the bill, you’ll receive a statement. We do offer payment plans. Please be aware that any outstanding amounts on your account might be sent to a collection agency for further action, so keeping your account current is important.

Yes, all information that we collect about you is confidential.

For eligibility purposes, household size encompasses mother, father, children, a significant other, husband, wife, and any dependent adult or child who relies on and benefits from the household’s collective income. Roommates are not considered “household members.” Additionally, unborn children are excluded from the household size calculation.

  • Children up to 18 years old, living at home, qualify as a dependent and their income will not count toward the families’ income

  • Children 18-21 years old, who are in college and still live at home, will qualify as a dependent and their income will not count toward the household income

  • Children over 18 years old, not in school and are working, will be considered on their own income and will not count as a dependent

  • Elderly parents living with their grown children can be counted as a dependent. If they have income it must be included in the household income otherwise they will be required to apply for the sliding fee program on their own.

The sliding fee begins on the day it’s submitted and remains active through December 31st of each year. Please allow up to 30 days from your application submission for processing and official approval. The sliding fee can be applied to earlier visits if the patient submitted their application using pay stubs or tax statements.

All services provided by Rubicon Healthcare are offered on a sliding fee scale. However, certain services might have a different nominal charge because they involve more expensive external providers. For additional information, please inquire at the front desk before receiving any services.
